Ozempic vs insulin — GLP-1 vs insulin therapy in type 2 diabetes: different roles, hypoglycemia/weight trade-offs, and when each is indicated.
Short answer: Insulin replaces the hormone your body lacks; Ozempic stimulates your own insulin release and reduces appetite. In type 2 diabetes, GLP-1s are preferred before insulin (weight benefit, low hypoglycemia). Insulin is essential when the pancreas can't produce enough — type 1 diabetes and advanced T2D.
Different jobs
Insulin = hormone replacement — it lowers glucose directly and is essential in type 1 diabetes and advanced T2D. Ozempic = incretin — amplifies your own insulin only when needed, plus weight/appetite effects insulin lacks (insulin causes weight gain).
Guideline preference
ADA guidelines position GLP-1s ahead of insulin in T2D escalation where weight/CV benefit matters — GLP-1s give better outcomes with less hypoglycemia. Insulin remains essential when β-cell function is depleted.
Sometimes both
Advanced T2D may need both — basal insulin for fasting control + GLP-1 for weight/CV benefit. Doses are adjusted carefully (hypoglycemia risk).
In T2D, often it defers or reduces insulin need. In T1D or advanced insulin deficiency, no — insulin is essential.
Insulin promotes weight gain; GLP-1s cause loss — that's why guidelines prefer GLP-1 first when appropriate.
